Social Worker (MSW) – Case Management – PRN

About the role

The Social Worker provides bio‑psychosocial assessments, patient education, and crisis interventions while coordinating complex discharge plans for patients at Ochsner Baptist. The role works under supervision of an LCSW and collaborates with a multidisciplinary team to address emotional, social, cultural, and spiritual needs.

Key responsibilities

• Conduct patient assessment interviews, provide referrals, and coordinate discharge plans.
• Identify and connect patients and families with community resources.
• Maintain patient records, document critical information, and communicate with the medical team.
• Participate in on‑call rotation and adapt communication style to diverse patient populations.

Required profile

• Master's degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); must obtain licensure within 6 months.
• Preferred: two years of experience in a medical setting and LMSW certification.

Required skills

• Proficiency with Windows‑style applications and data‑entry software.
• Working knowledge of medical terminology and common disease entities.
• Understanding of health‑care systems, Medicare/Medicaid, and public insurance programs.
