Social Worker – Cancer Center (PRN)

About the role

The Cancer Center seeks a PRN Social Worker to support patients and families through discharge planning, psychosocial assessment, and advocacy across the continuum of care.

Key responsibilities

• Manage discharge planning for assigned patients, ensuring timely discharge and connection to post‑discharge providers.
• Conduct initial and ongoing psychosocial assessments, provide counseling, and recommend interventions to reduce barriers to treatment.
• Educate patients, families, and physicians about post‑acute options, addressing financial and choice concerns.
• Advocate for patients in cases of abuse, domestic violence, and end‑of‑life issues.
• Serve as a resource, mentor, and preceptor for less experienced staff and represent the department on committees.

Required profile

• Master’s Degree in Social Work (MSW) or Master’s of Science in Social Administration (MSSA).
• Current state licensure as a Social Worker (LSW, LISW or LISW‑S).
• Basic Life Support (BLS) certification.
• Excellent communication and negotiation skills; ability to work independently and within multidisciplinary teams.
• Three years of healthcare system experience preferred, including hospital social work and knowledge of community resources.

Required skills

• Current state licensure as a Social Worker (LSW, LISW or LISW‑S).
• Basic Life Support (BLS) certification.
