Social Services Coordinator

About the role

The Social Services Coordinator assists patients in overcoming socioeconomic barriers to achieve health and wellness goals. This position ensures access to quality healthcare by connecting individuals with community resources, referral services, and life‑skill education.

Key responsibilities

• Provide individual and family counseling, referrals, and advocacy to improve personal and social functioning.
• Partner with Community Health Educators and agencies to identify and address unmet community needs and facilitate mutual referrals.
• Document patient contacts, case updates, and referrals according to department standards and reporting requirements.
• Track appointment compliance for care‑managed patients, prepare reports, and support audits as directed.

Required profile

• Associate degree in social services (Bachelor’s degree a plus).
• Two or more years of experience in social services or a related field.
• Strong verbal and written communication skills and ability to work harmoniously within a health‑care team.
• Fluency in Spanish is a plus.

Required skills

• Knowledge of community resources related to social determinants of health.
• Ability to maintain confidentiality of patient and family information.
