Senior Integrated Clinic Pharmacist

About the role

We are looking for a Senior Integrated Clinic Pharmacist to join our primary‑care organization and work remotely as a core member of the care team. You will support a full physician panel, improve medication outcomes for high‑risk patients, and collaborate across multiple clinic locations.

Key responsibilities

• Evaluate pharmacotherapy for chronic, high‑risk conditions (diabetes, hypertension, heart failure, COPD) using evidence‑based medicine.
• Execute clinical protocols within state and organizational regulations.
• Optimize drug coverage, select preferred formulary products, and manage cost‑effective alternatives.
• Collaborate with physicians, nurses, and multidisciplinary teams during high‑risk rounds.
• Provide patient counseling, disease‑state education, and post‑discharge medication reconciliation.
• Document all activities in the clinic EMR and support CMS Stars and HEDIS quality measures.
• Lead quality‑improvement initiatives, in‑service education, and ongoing pharmacotherapy updates.
• Respond to additional pharmacy‑related requests from the Primary Care Organization.

Required profile

• Active, unrestricted pharmacist license in Florida (willingness to obtain additional state licenses).
• Pharm.D. or equivalent advanced pharmacy degree.
• Minimum five years of practice experience as a pharmacist.
• Proven ability to work in a team‑oriented environment.

Required skills

• Clinical pharmacy expertise in chronic disease management.
• Medication therapy management and drug‑coverage optimization.
• Proficiency with electronic medical record (EMR) documentation.
• Strong knowledge of CMS Stars, HEDIS, and quality‑measure reporting.
