RN Hospice Coordinator – Main Campus & Eastside Region

About the role

The RN Hospice Coordinator provides continuity of care for patients transitioning from hospital to home or a facility, supporting Cleveland Clinic’s value‑based care model across the Main Campus and Eastside locations.

Key responsibilities

• Process inpatient hospice referrals, ensure continuity of care, and coordinate discharge planning for contract facilities.
• Educate patients, families, and potential customers about hospice benefits and eligibility.
• Order medical supplies and DME, hand off communication to field teams, and document all actions in the EMR.
• Perform admission assessments, daily patient assessments, pain assessments, and safety risk evaluations.
• Collaborate with interdisciplinary teams to develop and update care plans, attend meetings, and manage symptom control.

Required profile

• Graduate of an accredited nursing program with current RN licensure.
• At least one year of clinic experience and one year in an acute med/surg or critical‑care setting.
• Basic Life Support (BLS) certification.
• Knowledge of JCAHO and Medicare standards.
• Ability to work irregular hours, travel between hospitals and community sites, and lift up to 50 lb.
