RN Care Coordinator – Sickle Cell Adult & Pediatric

About the role

The RN Care Coordinator supports adult and pediatric patients with sickle cell disease at Cleveland Clinic Main Campus. Working a hybrid schedule, you will collaborate with multidisciplinary teams to improve treatment outcomes and reduce emergency department visits.

Key responsibilities

• Coordinate longitudinal care and disease management for high‑risk sickle cell patients.
• Tri­age patient concerns, ensure timely resolution, and involve appropriate providers.
• Conduct comprehensive clinical assessments covering medical, behavioral, pharmacy, social and end‑of‑life needs.
• Utilize registries, patient lists and care‑team tools to manage patient populations.
• Educate patients and families, coach self‑management, and serve as a liaison and advocate.

Required profile

• Graduate of an accredited nursing program and current RN licensure in the state.
• 3–5 years of nursing experience, preferably in hematology/oncology or pain management.
• Basic Life Support (BLS) certification.

Required skills

• Proficiency with care‑coordination technologies (registries, patient lists, care‑team dashboards).
• Strong clinical assessment and risk‑assessment abilities.
• Ability to identify barriers, connect patients to community resources, and facilitate transitions of care.
