RN Care Coordinator – Pulmonary Hypertension

About the role

The Cleveland Clinic Main Campus seeks a Registered Nurse Care Coordinator to manage pulmonary hypertension patients. The role involves longitudinal disease management, patient outreach, and coordination across multidisciplinary teams to improve outcomes and reduce readmissions.

Key responsibilities

• Collaborate with multidisciplinary teams to develop care plans and monitor patient progress.
• Conduct comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Provide education, coaching, and compliance monitoring for patients and families.
• Serve as liaison and advocate, facilitating transitions of care and connecting patients to community resources.

Required profile

• Graduate from an accredited nursing program with current RN licensure.
• Three to five years of nursing experience, preferably in high‑risk or specialty care.
• Basic Life Support (BLS) certification.
• Ability to perform physical tasks such as lifting up to 20 lb and using standard PPE.
