RN Care Coordinator – Pediatric Cardiology Fetal Care

About the role

The RN Care Coordinator will join Cleveland Clinic’s Main Campus to provide longitudinal care coordination for high‑risk pediatric cardiology patients, focusing on outreach, disease management, and patient education.

Key responsibilities

• Collaborate with multidisciplinary teams to develop care plans, set goals, and monitor patient progress.
• Identify patients needing ongoing coordination, conduct targeted outreach, and perform comprehensive clinical assessments.
• Educate patients and families, coach them on care plans, and monitor compliance.
• Serve as liaison and advocate, managing transitions of care and connecting patients to community resources.

Required profile

• Graduate of an accredited nursing program with current RN licensure.
• Three to five years of nursing experience, preferably in pediatric or high‑risk settings.
• Basic Life Support (BLS) certification.
• Ability to obtain cancer‑specific certification if working in oncology‑related areas.
