RN Care Coordinator – Adolescent Medicine, Hybrid

About the role

The RN Care Coordinator in Adolescent Medicine supports high‑risk pediatric patients and their families by coordinating care, providing education, and reducing unnecessary hospital utilization. The position works Monday‑Friday with a hybrid schedule, allowing one day per week to be performed from home.

Key responsibilities

• Collaborate with a multidisciplinary team to develop goals, plan interventions, and maximize patient outcomes.
• Identify patients needing ongoing coordination and conduct targeted outreach.
• Perform comprehensive clinical assessments covering medical, behavioral, pharmacy, social, and end‑of‑life needs.
• Educate patients and families, monitor compliance with care plans, and update plans as needed.
• Serve as liaison and advocate, managing transitions of care and ensuring clear communication across settings.
• Identify barriers, connect patients to community resources, and ensure adherence to disease‑specific care pathways.
• Participate in quality and process improvement initiatives within the practice.

Required profile

• Graduate of an accredited nursing program.
• Current RN licensure in the state of practice.
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience, preferably in pediatrics.
• Strong attention to detail and therapeutic communication skills.
