Registered Nurse – Home Health (Weekend)

About the role

The Home Health Hospice RN provides professional nursing care to terminally ill patients in their homes during weekend shifts. Working 12‑hour days on Saturday and Sunday, the RN collaborates with an interdisciplinary team to ensure comfort‑focused, end‑of‑life care.

Key responsibilities

• Conduct initial and ongoing assessments of physical, functional, psychosocial, and environmental needs.
• Develop, implement, and revise individualized care plans in coordination with the hospice clinical supervisor.
• Manage symptoms, prioritize comfort, and provide pain, pressure‑ulcer, and bowel‑impaction prevention.
• Educate patients and families about disease processes, self‑care, and end‑of‑life issues.
• Document clinical notes, communicate changes to providers, and act as liaison between the care team and families.
• Participate in on‑call duties and coordinate services through case‑management approaches.

Required profile

• Current Registered Nurse (RN) license.
• Experience in hospice or palliative home health care.
• Availability for weekend 12‑hour shifts and on‑call evenings.
• Ability to work independently in patients’ homes while maintaining interdisciplinary communication.

• Pressure ulcer, pain crisis, and bowel impaction prevention techniques.
