Registered Nurse - Home Health

About the role

Elite Home Health, part of LHC Group and the Optum family, offers home health, hospice and community-based care. As a Registered Nurse in Home Health you provide and direct nursing care to patients in their homes as prescribed by the physician, in line with laws, regulations and agency policies, and coordinate the total plan of care with other health care professionals.

Key responsibilities

• Initiate, develop, implement and revise the plan of care with the physician and other health care professionals
• Supervise home health aides and LPNs/LVNs, give instruction and assign tasks according to state and federal regulations, and carry out required supervisory visits
• Deliver high-quality clinical care within scope of practice and infection control standards, and complete OASIS assessments including medication reconciliation
• Document patient visits per policy and payer requirements
• Make initial and comprehensive nursing evaluations, check eligibility and medical necessity, and submit accurate documentation
• Communicate with agency staff, physicians, patients and families about care issues, visit assignments, schedule changes, orders, OASIS data, coding requests and coordination with other clinicians
• Follow the Clinical Director's and Executive Director's direction, assist in orienting new staff and act as preceptor, and take part in survey readiness, performance improvement and on-call and weekend rotations
• Work within the agency's utilization management model and adapt to any home situation

Required profile

• Current, unrestricted RN license in the state of practice
• Current driver's license, vehicle insurance and access to a reliable vehicle or public transport
• Preferred: current CPR certification or ability to obtain it within 90 days of hire
• Preferred: 1+ year of home health experience
• Ability to work independently

What we offer

• Total cash compensation of $71,677 to $107,516 annually, based on full-time (40 weekly hours) and full productivity, made up of per-visit point pay and hourly pay
