Registered Nurse - Home Health

About the role

Assured Home Health, a part of LHC Group and a member of the Optum family, is hiring a Registered Nurse for home health in Ephrata, Washington. The nurse provides and directs nursing care to patients in their homes as prescribed by the physician, coordinates the total plan of care with other health care professionals, and helps maintain continuity of care through information exchange with physicians, agency staff, patients, families and community resources. A $5,000 sign-on bonus is available for qualified full-time candidates.

Key responsibilities

• Initiate, develop, implement and revise the plan of care with the physician and other health care professionals
• Supervise care provided by home health aides and licensed practical/vocational nurses, provide instruction and assign tasks according to state and federal regulations, and carry out required supervisory visits
• Deliver high-quality clinical services within the scope of practice and infection control standards, and complete comprehensive OASIS assessments including medication reconciliation
• Document patient visits per policy and payer requirements, and submit accurate documentation for quality evaluations and eligibility checks
• Communicate with physicians, patients and families, and with agency staff about care issues, visit assignments, schedule changes, orders, OASIS data sets and coding requests
• Take direction from the Clinical Director and Executive Director, assist in orienting new staff and act as a preceptor, and participate in survey readiness and performance improvement activities
• Work on-call and rotate weekends as needed, follow the utilization management model, and reduce unnecessary hospitalizations

Required profile

• Current and unrestricted RN licensure in Washington
• 1+ year of home health experience
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation
• Current CPR certification
• Ability to work in any home situation regardless of the client's background

Preferred qualifications

• Ability to work independently
• Solid communication, writing and organizational skills

What we offer

• Total cash target pay of $93,000-$124,800 for full-time work (40 weekly hours), made up of per-visit point pay and hourly pay for non-visit activities
