Qualified Mental Health Professional (Behavioral Health Consultant)

About the role

Provides brief and ongoing assessment, intervention, and onsite, community or telehealth consultation to the primary care team regarding psychiatric and psychosocial concerns. The role focuses on delivering evidence‑based behavioral health interventions within primary care to support health behavior change.

Key responsibilities

• Deliver on‑site and telehealth behavioral health services in the primary‑care clinic.
• Maintain a schedule that allows immediate access by primary‑care providers and promptly respond to calls and inquiries.
• Conduct initial assessments and brief cognitive‑behavioral interventions for patients referred by providers.
• Use evidence‑based practices, screening tools, and crisis‑intervention protocols as needed.
• Provide immediate verbal or written feedback to providers and document follow‑up interventions.
• Develop and maintain resource lists, facilitate referrals, and contribute to program development with clinic leadership.

Required profile

• Master’s degree in psychology, social work, or counseling.
• Qualified Mental Health Assistant (QMHA) certification.
• Registered in Oregon as an LPC Associate or Clinical Social Work Associate (CSWA).
