Psychiatric Nurse Practitioner – Primary Care/Behavioral Health

About the role

At Revere Health, the Psychiatric Nurse Practitioner (PMHNP) joins an integrated primary‑care and behavioral‑health team to provide comprehensive mental‑health services directly within the primary‑care setting. The role focuses on timely assessments, medication management, and collaborative treatment planning to ensure whole‑person care.

Key responsibilities

• Conduct psychiatric evaluations for patients referred by primary‑care providers and the behavioral‑health team.
• Provide medication management for common mental‑health conditions encountered in primary care.
• Deliver brief, supportive clinical interventions aligned with integrated behavioral‑health workflows.
• Participate in warm‑hand‑offs, joint visits, daily huddles, case reviews, and Collaborative Care‑style meetings.
• Coordinate with care managers, nurses, and social workers to ensure follow‑up, safety planning, and referrals.
• Maintain clear communication of clinical findings, treatment plans, and patient progress across the care team.
• Facilitate referrals to specialty psychiatric services and assist with transitions of care, including hospital discharges.
• Use standardized screening tools (PHQ‑9, GAD‑7, ADHD scales) to guide diagnosis, monitor outcomes, and support value‑based care goals.

Required profile

• Master’s or Doctorate degree in Nursing with PMHNP certification.
• Active Utah APRN license (or ability to obtain before start date).
• DEA license with prescribing authority.
• Experience or strong interest in working within a primary‑care and behavioral‑health integrated model.

Required skills

• Proficiency with standardized mental‑health screening tools such as PHQ‑9, GAD‑7, and ADHD scales.
• Medication management and prescribing for psychiatric conditions.
