Practice Transformation Coach

About the role

The Practice Transformation Coach acts as a strategic advisor and subject‑matter expert, driving clinical quality improvement and operational performance across provider practices in Astrana’s value‑based care network in the Maryland and Northern Virginia region.

Key responsibilities

• Assess clinical, operational, and financial performance of provider practices and identify improvement opportunities.
• Prioritize engagements and design customized transformation plans to boost quality metrics, workflow efficiency, and financial outcomes.
• Lead workflow redesign initiatives, including EHR optimization, clinical documentation, and billing practices.
• Analyze complex performance data (HEDIS, pay‑for‑performance, medical cost trends) and translate insights into actionable strategies.
• Collaborate with internal stakeholders in Market Operations, Quality, and Population Health to align improvement initiatives.
• Serve as a liaison between practices and organizational leadership, escalating risks and sharing insights.

Required profile

• Experience advising provider practices on clinical quality and operational performance.
• Ability to exercise independent judgment in assessing practice needs and prioritizing interventions.
• Strong analytical skills to interpret performance data and develop data‑driven solutions.
• Proven capability to influence provider decision‑making and guide workflow redesign.
