Physical Therapist – SNF West Campus (PRN)

About the role

Ochsner Health is seeking a Physical Therapist to join the SNF West Campus team on a PRN basis. The role involves evaluating patients and delivering inpatient physical therapy programs based on physician orders, while collaborating with the broader healthcare team to ensure optimal discharge planning.

Key responsibilities

• Perform patient evaluations and develop individualized treatment plans.
• Monitor patient response, adjust treatment plans, and document progress according to hospital standards.
• Communicate effectively with the multidisciplinary team to facilitate appropriate post‑acute or home care.
• Maintain accurate patient records and adhere to safety and quality protocols.

Required profile

• Bachelor’s degree in Physical Therapy.
• Current state physical therapy license or eligibility for licensure.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong judgment, decision‑making abilities, and interpersonal skills.

Required skills

• Proficiency with computers, software, and web‑based applications.
• Current physical therapy licensure.
• Basic Life Support (BLS) certification.
