Physical Therapist – Per Diem Weekends (Home Health)

About the role

We are seeking a compassionate Physical Therapist to provide per‑diem weekend care to patients in Springfield, MO and surrounding areas. This role allows you to deliver clinical services in the comfort of clients’ homes while enjoying a flexible schedule.

Key responsibilities

• Visit clients in their homes to assess and treat injuries, illnesses, surgeries, or post‑hospitalization needs.
• Perform diagnostic tests such as range‑of‑motion, strength, balance, gait, and safety assessments.
• Develop and adjust individualized care plans that include manual therapeutic exercises, gait training, and balance interventions.
• Educate patients and families on rehabilitative activities to promote independence and safety.
• Document all observations, interventions, and evaluations using a tablet‑based system.

Required profile

• Active and current Physical Therapist license.
• At least one year of clinical experience preferred.
• Willingness to travel locally to assigned cases.
• Previous home‑health experience is a plus but not required; training will be provided.

Required skills

• Ability to conduct mobility, strength, and balance assessments.
• Proficiency in developing and implementing therapeutic exercise programs.
• Knowledge of wound‑care protective interventions.
