About the role

Join a flexible, field‑based team delivering pediatric preventive care directly to families across Coconino County, Arizona. You will set your own schedule and perform in‑home well‑child visits, helping children stay healthy and connected to follow‑up care.

Key responsibilities

• Conduct scheduled in‑home Well‑Child Visits throughout Coconino County.
• Review pediatric medical history, risk factors, vitals and perform age‑appropriate screenings.
• Complete preventive assessments and accurately document findings in the EMR.
• Educate patients and families on recommended next steps and coordinate follow‑up care.
• Identify care gaps, manage your local route and operate independently without daily clinic reporting.

Required profile

• Active Nurse Practitioner license in Arizona.
• Registered/enrolled with Arizona Medicaid.
• PNP‑PC certification or equivalent pediatric NP certification.
• Valid driver’s license and reliable transportation.
• Comfortable working independently in patients’ homes.
