Patient Navigator

About the role

The University of Miami Health System is seeking a part‑time Patient Navigator to join the Sylvester Cancer Center. The role supports patients by identifying barriers to care, coordinating treatment plans, and ensuring follow‑up with providers.

Key responsibilities

• Conduct patient evaluations to gather information for effective treatment planning.
• Facilitate access to insurance and eligible social service programs.
• Provide appointment reminders and act as liaison between patients and providers.
• Identify and resolve real or perceived barriers to care.
• Educate and counsel patients on health and life‑situational issues.
• Maintain up‑to‑date knowledge of community health and welfare resources.
• Handle patient communications, obtain medical records, and address care gaps with the nurse navigator.
• Prepare, track, and process medical records and pathology specimens while ensuring confidentiality.

Required profile

• High school diploma or equivalent.
• Minimum one year of relevant work experience.
• Ability to handle confidential information with discretion.
• Strong oral and written communication skills.
• Capacity to manage stressful situations with professional composure.
• Effective interpersonal relationship skills and ability to follow instructions.
