Part-Time Nurse Practitioner – Clinical Lead for ACCESS Program

About the role

We are seeking a forward‑thinking Nurse Practitioner to serve as the Clinical Lead for our ACCESS program, a virtual care initiative that integrates connected health devices, AI insights, and a dedicated care team. This part‑time, 1099 contract role will help shape clinical workflows, protocols, and standards for managing cardiometabolic conditions at scale.

Key responsibilities

• Conduct virtual medical evaluations via video, phone, and asynchronous forms, confirming diagnoses and documenting baseline metrics.
• Create and manage evidence‑based care plans for obesity, dyslipidemia, hypertension, ASCVD, mild‑to‑moderate CKD, and pre/diabetes.
• Prescribe and titrate medications in Full Practice Authority states; collaborate with the Medical Director in other states.
• Manage device‑triggered Red Alerts (e.g., BP > 180/110), triaging patients for ER referral or immediate medication adjustment.
• Document all patient interactions in the EHR with accurate ICD‑10 coding and capture comorbidities for proper reimbursement.
• Prepare coordination reports for referring PCPs and maintain partnership with external clinicians.
• Collaborate with the Medical Director to develop titration algorithms, refine clinical protocols, and improve app‑based care tools.

Required profile

• Active Nurse Practitioner license in good standing; IMLC/eNLC compact license preferred.
• Board certification through ANCC or equivalent.
• Willingness to obtain licensure in additional target states within 90 days of start.
• Experience delivering virtual care and managing chronic cardiometabolic conditions.

Required skills

• EHR documentation with accurate ICD‑10 coding.
