Nurse Practitioner – Value-Based Care in Gainesville/Buford, GA

About the role

Upperline Health is seeking a Nurse Practitioner to deliver clinic‑based, home, and telehealth care for adult patients with chronic conditions in its Gainesville and Buford, GA locations. The role works closely with physicians and an interdisciplinary team to develop and execute value‑based treatment plans.

Key responsibilities

• Assess patients, obtain histories, perform physical exams and order diagnostics in clinic, home, or via telehealth.
• Develop and implement individualized care plans for conditions such as diabetes, COPD, CHF, dementia and other chronic illnesses.
• Collaborate with physicians, pharmacists, dietitians, social workers and other specialists to coordinate comprehensive care.
• Educate patients and families on medication management, nutrition, and preventive strategies.
• Communicate clinical information to patients’ primary physicians and the broader Upperline care team.

Required profile

• Current Nurse Practitioner license in Georgia.
• Experience in primary care, internal medicine, geriatrics, home‑based care, palliative or hospice settings, or a value‑based care environment.
• Demonstrated compassion, attention to detail and ability to thrive in a collaborative, fast‑growing team.
