About the role

The Nurse Navigator 1 (H) serves as the first point of contact for patients entering the health system, ensuring timely scheduling of initial appointments and coordinating care throughout the treatment continuum. The role acts as a liaison among patients, families, caregivers, and a multidisciplinary care team, promoting high‑quality, patient‑centered care.

Key responsibilities

• Triages new patient appointments to appropriate providers and secures timely initial scheduling.
• Reviews external medical records and assesses barriers to care, referring patients to support services as needed.
• Educates patients on treatment plans, supports them throughout the care continuum, and provides counseling on positive health practices.
• Collaborates with multidisciplinary experts to determine optimal treatment pathways and documents patient encounters in EPIC.
• Mentors new staff, participates in continuing education, and contributes to process‑improvement initiatives.

Required profile

• Bachelor’s degree in a relevant field.
• Valid State of Florida Registered Nurse (RN) license.
• Minimum two years of relevant work experience.
• Strong interpersonal and communication skills, both oral and written.

Required skills

• Proficiency with Microsoft Office suite.
• Experience using the EPIC electronic health record system.
