New Grad Occupational Therapist – Full Time Home Health

About the role

Join Integrity Home Care + Hospice as a full‑time Occupational Therapist providing 1:1 care in patients' homes across Springfield, MO and surrounding areas. Enjoy flexible scheduling, hands‑on training, and 24/7 clinical support while helping clients achieve greater independence.

Key responsibilities

• Conduct home visits and perform comprehensive functional evaluations, including muscle function, endurance, visual coordination, communication skills, ADL and IADL performance.
• Develop and adjust individualized intervention plans based on cognitive, psychological, and motor assessments.
• Assess home environments for hazards, recommend equipment, and implement environmental adaptations.
• Collaborate with nursing and multidisciplinary teams to integrate occupational therapy into overall care plans.
• Educate patients, families, and caregivers on rehabilitative activities and safety practices.
• Document all observations, interventions, and outcomes using a tablet‑based electronic medical record system.

Required profile

• Current Missouri Occupational Therapist license.
• Graduation from an accredited OT program (CAHO/AMA and AOTA).
• Ability to work independently, manage time effectively, and travel locally to client homes.
• Strong interpersonal, organizational, and problem‑solving abilities.

Required skills

• Proficiency with electronic medical record (EMR) systems.
• Solid computer skills, including use of tablet devices for documentation.
