Licensed Practical Nurse (LPN) – Outpatient Care

About the role

Supervised by a Registered Nurse or Physician, the LPN guides patients through a seamless process to ensure high‑quality, efficient care. The LPN directs patients through appointments, provides nursing services during care, and performs phone triage.

Key responsibilities

• Direct patients through appointments and provide nursing services during care.
• Perform phone triage and determine urgency of care needs.
• Collaborate in care‑planning discussions with RN, MD, or other providers.
• Triage emergent care situations to the appropriate provider.
• Schedule acute‑care appointments to maximize patient care opportunities and unit efficiency.
• Maintain provider schedule flow during patient care sessions to minimize wait times.
• Occasionally work evenings and weekends as required.

Required profile

• Licensed Practical Nurse in New York State.
• Minimum one year of LPN experience.
• Outpatient primary‑care and/or pediatric experience preferred.
• Current American Heart Association CPR and AED certification.
• Must successfully pass a medication administration proficiency exam.
