Licensed Practical Nurse – Home Health

About the role

BAYADA Home Health Care seeks a compassionate Licensed Practical Nurse to provide skilled nursing and rehabilitative care to adult clients in homes across Windsor and Windham County, Vermont.

Key responsibilities

• Make home visits to clients in the designated territory.
• Administer medication, perform wound care, treatments, and procedures.
• Monitor client conditions and report changes to the Clinical Manager and Client Services Manager.
• Document observations, interventions, and evaluations using electronic medical records on a tablet.

Required profile

• Current LPN license in Vermont.
• At least one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Ability to work independently, manage time effectively, and travel to assigned cases.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation and communication.
