Licensed Practical Nurse – Full‑time, Part‑time, PRN

About the role

Commonwealth Home Care is seeking Licensed Practical Nurses (LPNs) to provide skilled, compassionate care to clients in their homes across the Peninsula/Hampton Roads area. Positions are available as full‑time, part‑time, and PRN (per diem) assignments.

Key responsibilities

• Provide skilled nursing care under the direct supervision of a Registered Nurse.
• Assist in creating, implementing, and evaluating individualized care plans.
• Perform specialized procedures and teach clients self‑care techniques.
• Observe, record, and report clients’ physical and mental conditions and any changes.
• Participate in weekly case conferences, in‑services, and staff meetings.

Required profile

• Current LPN license in good standing with the Virginia Board of Nursing.
• Minimum one year of LPN experience; home‑care experience preferred.
• Ability to provide two professional references.
• Pass OIG, Virginia State Police, and national background checks.
• Current CPR certification, reliable transportation, valid driver’s license and insurance.

Required skills

• Skilled nursing techniques appropriate for home health settings.
• Basic documentation and reporting of client status.
• Ability to follow RN supervision and care‑plan directives.
