Lead Nurse Care Coordinator (RN) – Remote, CA License

About the role

Clarity Pediatrics is seeking a Lead Nurse Care Coordinator (RN) to join its fully remote interdisciplinary team. The role focuses on coordinating care for children with chronic conditions, ensuring seamless communication between families, providers, and pharmacies while supervising a team of nurses, medical assistants, and community health workers.

Key responsibilities

• Recruit, hire, train, and onboard new care coordination staff.
• Provide direct supervision, performance management, and professional development for the team.
• Maintain compliance with HIPAA, state nursing regulations, and other healthcare standards.
• Coordinate clinical appointments, referrals, and follow‑up to reduce gaps in care and no‑show rates.
• Manage medication prior authorizations, pharmacy communications, and accurate medication records.
• Communicate clinical updates to referring physicians and support providers during escalations of care.
• Participate in quality‑improvement initiatives and track key performance metrics.

Required profile

• Active California RN license in good standing.
• Proven experience leading a team of clinical staff.
• Strong organizational, communication, and interpersonal skills.
• Ability to thrive in a fully remote, virtual environment.
• Comfort working with diverse, medically and socially complex pediatric populations.
• Startup mindset with enthusiasm for iterating new care models.
• Reliable internet connection and dedicated workspace.
• Bilingual proficiency in English and Spanish.

Required skills

What we offer

• Salary range $90,000 – $110,000 per year.
• Medical and ancillary benefits (varies by location).
• Full‑time schedule: Monday‑Friday, 9 AM‑6 PM PST with occasional Saturday coverage.
