Full-Time Home Health Registered Nurse

About the role

Integrity Home Care + Hospice is seeking a full‑time Registered Nurse to join its home health visit team in Springfield, MO and surrounding areas. You will provide skilled nursing and rehabilitative care in patients' homes, enabling them to remain safely at home while receiving high‑quality care.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Provide acute and post‑acute care, including management of CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document wound assessments, care plans, and other clinical information accurately.

Required profile

• Current Missouri nursing license in good standing.
• At least one year of recent hands‑on experience with adult/geriatric patients in acute, emergency, home health, hospice, ICU, med‑surg, sub‑acute, or rehab settings.
• Ability to work independently, make real‑time clinical decisions, and prioritize urgent visits.
• Strong problem‑solving skills and cultural sensitivity.

Required skills

• Head‑to‑toe assessment proficiency.

• Point‑of‑care documentation and technology use.
