Community Health Worker Workleader

About the role

The Community Health Worker Workleader joins Cleveland Clinic Health System to lead and train a team of Community Health Workers. This position oversees daily operations, ensures quality documentation, and supports a positive patient experience across multiple sites.

Key responsibilities

• Provide leadership, coaching, and training to assigned Community Health Workers.
• Oversee daily operations, conduct quality reviews of monthly OKRs and performance metrics.
• Support completion of EPIC documentation and ensure accurate record‑keeping.
• Travel up to 25% to supervise onsite locations, including OB practices and the Fairview Wellness site.
• Facilitate cultural mediation, health literacy education, and cross‑cultural communication for patients and communities.
• Develop and maintain relationships with community agencies, making referrals as needed.
• Participate in evaluation and research activities related to CHW services.

Required profile

• High school diploma or GED.
• Community Health Worker certification through the Board of Nursing.
• Valid driver’s license.
• At least four years of experience as a Community Health Worker or patient navigator.
• Strong time‑management skills and ability to balance multiple priorities.
• Effective oral and written communication with diverse populations.
• Ability to consider cultural, social, and behavioral factors in public health service delivery.
