Clinical Pharmacist – Transplant (PRN)

About the role

The Clinical Pharmacist – Transplant (PRN) provides decentralized pharmacy services for the inpatient Lung Transplant Program at Cleveland Clinic Main Campus. The pharmacist works with transplant physicians, advanced practice providers, nurses, and the multidisciplinary team to optimize medication therapy for complex transplant patients throughout their inpatient stay.

Key responsibilities

• Verify medication orders and dispense medications accurately in compliance with state and federal regulations.
• Manage immunosuppressive therapy, perform therapeutic drug monitoring, and provide individualized pharmacotherapy recommendations.
• Document pharmacy services promptly and ensure compliance with national accreditation standards.
• Educate and train students, residents, other caregivers, and patients on medication use.

Required profile

• PharmD or Bachelor’s of Science in Pharmacy from an accredited college.
• Current state licensure as a Registered Pharmacist (RPh) or eligibility for reciprocity within 90 days of hire.
• Current Basic Life Support (BLS) certification.
• Completion of a Board of Pharmacy‑approved immunization administration course or certification within 90 days of hire (if administering immunizations).

Required skills

• Knowledge of immunosuppressive therapy management and therapeutic drug monitoring.
• Ability to verify and dispense medications accurately.
• Proficiency in documenting clinical pharmacy services.
