Clinical Pharmacist – Internal Medicine

About the role

The Clinical Pharmacist in Internal Medicine is responsible for medication management of assigned patients, ensuring appropriate therapy based on health status, institutional guidelines, and cost‑effective measures. The role includes medication reconciliation, monitoring, patient education, and active participation in resident and staff education.

Key responsibilities

• Provide pharmacokinetic consultation, using laboratory data to select and dose medications and communicate findings to healthcare providers.
• Evaluate patient response to drug therapy, document interventions, and conduct outcome studies.
• Develop pathways, best practices, and guidelines for the assigned patient population.
• Educate staff, students, patients, and other healthcare professionals; mentor residents and new staff.
• Participate in order set development, benefit design, formularies, and other pharmacy‑related documents.

Required profile

• Completion of a PGY‑1 Pharmacy Practice Residency (required).
• Current Louisiana pharmacist license.
• Effective verbal and written communication skills.
• Strong leadership and teaching abilities.
