Clinical Pharmacist – Critical Care, 7‑on/7‑off Nights

About the role

The Clinical Pharmacist will provide decentralized pharmaceutical care for critically ill patients on the night shift at Cleveland Clinic Main Campus. Working a 7‑on/7‑off schedule, the pharmacist supports ICU medication management, emergency response and central pharmacy operations.

Key responsibilities

• Coordinate and deliver medication services for ICU patients during night hours.
• Verify medication orders, review high‑risk drugs and provide dosing recommendations such as vancomycin optimization.
• Participate in emergency and stroke alerts, serving as a drug‑information resource.
• Support central pharmacy operations, including the Emergency Department and stroke response.
• Engage in medication safety and quality initiatives and educate students, residents and other caregivers.

Required profile

• PharmD or BS in Pharmacy from an accredited program.
• Current state licensure as a Registered Pharmacist (or eligibility within 90 days).
• Completion of a PGY‑1 residency or two years of acute/critical care pharmacy experience.
• Preferred: PGY‑2 residency, fellowship, or 3‑4 years of direct critical‑care experience.
