Clinical Pharmacist – Cardiology

About the role

The Clinical Pharmacist – Cardiology supports the Heart, Vascular & Thoracic Institute at Cleveland Clinic Main Campus, caring for complex, high‑acuity cardiology patients across multiple subspecialties. The role combines clinical and operational responsibilities in a hybrid centralized/decentralized model.

Key responsibilities

• Review and interpret medication orders, identify and resolve drug‑related problems, and provide evidence‑based recommendations to providers.
• Precept pharmacy residents and students, and educate pharmacists, nurses, and other caregivers.
• Participate in quality improvement initiatives, drug shortage management, and process improvement projects.
• Collaborate with multidisciplinary teams on high‑complexity patient cases and contribute to formulary decisions.

Required profile

• PharmD or BS in Pharmacy from an accredited program.
• PGY1 residency plus at least 2 years of inpatient clinical pharmacy experience with ≥50% time in cardiology, or 4 years of equivalent experience.
• Current state licensure as a Registered Pharmacist (or eligibility for reciprocity within 90 days).
• Basic Life Support certification; ACLS or PALS certification as required.

Required skills

• Clinical expertise in cardiology pharmacotherapy.
• Ability to perform pharmacotherapeutic monitoring and medication therapy management.
• Proficiency in educating and precepting pharmacy trainees.
