Case Management Social Worker – Neuro Expansion

About the role

The Cleveland Clinic Neurological Institute is expanding its state‑of‑the‑art facility and seeks a dedicated Case Management Social Worker to join interdisciplinary teams. You will provide patient‑centered support across the neuro continuum, from ICU to inpatient units, ensuring smooth transitions and resource navigation.

Key responsibilities

• Manage discharge planning for assigned patients, coordinating timely transitions to post‑acute care providers.
• Conduct psychosocial assessments, provide counseling, and intervene on issues such as abuse, domestic violence, and end‑of‑life decisions.
• Educate patients, families, and physicians about post‑acute options, addressing financial concerns and freedom of choice.
• Advocate for patients by connecting them with community resources and eliminating barriers to care.

Required profile

• Master’s degree in Social Work or Social Administration and current state licensure (LSW, LISW, LISW‑S).
• Minimum three years of social work experience in healthcare settings, with strong knowledge of pre‑acute and post‑acute care.
• Basic Life Support (BLS) certification through AHA or Red Cross.

Required skills

• Proficiency in crisis intervention and care coordination.
• Ability to navigate community resources and complete accurate documentation.
