Case Management Social Worker – Float

About the role

The Case Management Social Worker (Float) provides psychosocial support across multiple adult inpatient units at Cleveland Clinic Main Campus. This full‑time, day‑shift position rotates weekends and holidays, offering exposure to diverse patient populations and interdisciplinary teams.

Key responsibilities

• Conduct comprehensive psychosocial assessments and develop interventions to reduce barriers to care.
• Manage discharge planning, coordinate post‑acute options, and educate patients, families, and physicians.
• Advocate for patients in cases of abuse, domestic violence, and other safety concerns.
• Collaborate with case managers, medical staff, and community resources to ensure continuity of care.

Required profile

• Master’s Degree in Social Work or Master’s of Science in Social Administration.
• Current state licensure as a Social Worker (LSW, LISW, LISW‑S, or PC).
• Basic Life Support certification (BLS) through AHA or Red Cross.
• 3+ years of experience in a healthcare setting, including pre‑acute and post‑acute care.
