Case Management Social Worker – Epilepsy

About the role

The Social Worker will support individuals and families within the Adult Behavioral Health Center’s Epilepsy program, providing crisis intervention, resource coordination, counseling, and patient empowerment across inpatient and outpatient settings.

Key responsibilities

• Collaborate with multidisciplinary teams to assess and plan interventions for social, emotional, financial and environmental challenges.
• Conduct psychosocial assessments and deliver counseling, education, advocacy, referrals, conflict resolution, crisis intervention and problem‑solving.
• Document treatment recommendations, care‑coordination interventions and develop care plans.
• Engage patients and families to reduce barriers and promote adherence to treatment plans.
• Maintain a network of community resources and connect patients to appropriate services.
• Supervise Social Work Interns and assist in training new caregivers.
• Educate staff on the impact of social problems on health outcomes.

Required profile

• Master’s Degree in Social Work from a CSWE‑accredited program.
• Current state licensure as a Licensed Social Worker (LSW) and eligibility for Licensed Independent Social Worker (LISW) within five years.
• At least 3 years of experience in a healthcare system, preferably with hospital social work and psychiatric experience.
• Strong communication and negotiation skills for effective multidisciplinary collaboration.
• Knowledge of community resources and ability to maintain a resource network.
