Case Management Social Worker – Behavioral Health & Psychiatry Liaison

About the role

The Case Management Social Worker supports patients with complex psychosocial needs on the Behavioral Health Emergency Response Team (BERT) and as a Psychiatry Consult Liaison. Working Monday‑Friday, 8:00 am‑5:00 pm on the Cleveland Clinic Main Campus, the role focuses on assessment, crisis intervention, discharge planning and coordination of community resources.

Key responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized support plans.
• Coordinate care and services for high‑risk patients, including crisis intervention and safe discharge planning.
• Collaborate with interdisciplinary teams, families, and community organizations to address barriers to care.
• Provide counseling, education on post‑acute options, and advocacy for patients experiencing abuse, domestic violence, or other safety concerns.
• Serve as a preceptor and mentor to less experienced caregivers and represent the department on committees.

Required profile

• Master’s degree in Social Work (MSW) or Master of Science in Social Administration (MSS).
• Current state licensure as a Social Worker (LSW, LISW, or LISW‑S).
• Basic Life Support (BLS) certification.
• Flexible, open‑minded, and adaptable to change.
• Preferred: Case Manager certification within one year, three years of healthcare system experience, hospital social work background, and knowledge of community services.
