Care Navigator (Texas – Part‑Time)

About the role

Nolia Health is seeking a remote Care Navigator to support patients and family caregivers dealing with chronic conditions and serious illness related to aging. The role is virtual, part‑time, and focuses on providing biopsychosocial support, personalized care planning, and connection to community resources.

Key responsibilities

• Serve as the primary point of contact for caregivers and families, conducting biopsychosocial assessments and developing culturally responsive care plans.
• Provide disease‑specific education, refer to community services, and coordinate with primary care physicians and specialists.
• Deliver caregiver education on burnout, self‑care, and resilience, and maintain accurate documentation of all interactions.
• Participate in monthly transdisciplinary meetings and represent Nolia Health at community events to generate referrals.

Required profile

• At least 2 years of experience in a healthcare or community‑based setting.
• Understanding of family caregiver challenges and cultural humility.
• Ability to manage emotionally charged interactions and work independently in a remote environment.

Required skills

• Experience with Electronic Health Records (EHR) systems.
• Strong general computer skills and ability to troubleshoot minor technology issues.
