Care Management Assistant

About the role

Join Cleveland Clinic’s Main Campus where research, advanced surgery, and world‑class patient care converge. As a Care Management Assistant you will facilitate the transition of patients to post‑acute care and support the procurement of supplies needed for discharge plans. You will coordinate with extended‑care facilities, ensure clinical documentation is provided, arrange transportation, and document all transition details.

Key responsibilities

• Participate in discharge planning for complex patients and partner with Transitional Care Coordinators and Social Workers.
• Communicate bed availability at extended‑care facilities and advise care teams.
• Transmit patient demographic information and coordinate regulatory paperwork (PASS‑ID, etc.) before transitions.
• Prepare medical record information and arrange transportation to post‑acute settings.
• Maintain ongoing communication with care coordinators regarding transition status.
• Assist in procuring hard‑to‑find medications, medical supplies, and other discharge items.

Required profile

• High School Diploma or GED.
• Three years of recent experience working in healthcare or a related field.
• Fluency in medical terminology.
• Computer literacy.
