Patient Care Pharmacist - Short term remote assignment

About the role

Fuze Health is seeking a remote Patient Care Pharmacist to join its Specialty Operations team on a short‑term basis. The pharmacist will support the Fuze Rx Patient Management Program, ensuring safe, accurate, and efficient dispensing while delivering patient‑first clinical care.

Key responsibilities

• Provide patient consultations via text, app messaging, email, or phone.
• Conduct initial assessments of chart notes to verify appropriate prescribing patterns.
• Perform clinical check‑ins to monitor adherence, efficacy, adverse reactions, and drug interactions.
• Identify patients needing clinical interventions and collaborate with providers on therapeutic recommendations.
• Facilitate transition of care by transferring prescriptions to appropriate pharmacies when necessary.
• Communicate prescription status updates to patients and providers.
• Maintain deep knowledge of pharmacy billing across Medicaid, Medicare, ADAP, commercial insurance, and foundation assistance programs.
• Proactively identify and address patient cost barriers.

Required profile

• Licensed pharmacist with experience in specialty pharmacy operations.
• Strong alignment with a patient‑first culture and ability to work autonomously.
• Passion for evaluating and improving internal processes and SOPs.
• Comfortable collaborating on a multi‑state team and engaging with cross‑functional partners.
