About the role
This position leads the design, build, and scaling of a technology‑enabled clinical pharmacy program within a fast‑growing virtual care platform. You will act as both a hands‑on pharmacist and a strategic program builder, shaping pharmacy services that improve medication outcomes across a national care network.
Key responsibilities
- Design, build, and scale the clinical pharmacy program, including service scope, care delivery model, and operational infrastructure.
- Provide direct clinical pharmacy services such as medication reconciliation, therapy reviews, and drug interaction identification.
- Support patients with medication adherence, safety education, and health literacy during virtual and in‑home care episodes.
- Collaborate with clinical leadership, product, and technology teams to define pharmacy scope, shape workflows, and integrate EMR tools.
- Monitor prescription‑related questions, handle clinical escalations, and contribute to preventive care initiatives (HEDIS, care‑gap closure).
- Ensure regulatory compliance with state and federal pharmacy requirements.
- Document clinical interventions in EMR systems for reporting and analytics.
- Assist in hiring, onboarding, and developing a growing pharmacy team.
Required profile
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Active pharmacist licensure (multi‑state preferred).
- 5+ years of clinical pharmacy experience with a track record of independent work and leading initiatives.
- Proven experience building or scaling pharmacy programs or clinical teams.
- Experience in telehealth, virtual care, home health, or value‑based care environments.
- Strong knowledge of medication therapy management and HEDIS pharmacy measures.
- Excellent communication and collaboration skills with multidisciplinary teams.
