Certified Nursing Assistant – Home Health (Waukesha & Lake Country)

About the role

Here at BrightStar Care of Greater Waukesha and Lake Country we are seeking a dedicated Home Health Certified Nursing Assistant (CNA) to provide hands‑on support to patients in their homes. The role focuses on maintaining comfort, independence and safety for clients while following individualized care plans.

Key responsibilities

• Provide personal care such as bathing, grooming, dressing and toileting.
• Assist with mobility, transfers and safe ambulation.
• Support activities of daily living including meal preparation and light housekeeping.
• Monitor and report changes in patient condition to the supervising nurse.
• Offer companionship and emotional support.
• Follow individualized care plans and maintain accurate documentation.
• Ensure a safe, clean environment for each patient.

Required profile

• Active Wisconsin CNA certification.
• Current CPR certification (preferred).
• Reliable transportation, auto insurance and a valid driver’s license.
• Experience in home health or caregiving (preferred).
• Strong communication skills and a compassionate, patient‑centered approach.
• Ability to work independently and maintain professionalism in a home setting.
