RN Case Manager

About the role

The RN Case Manager at Ochsner Health coordinates multidisciplinary communication and facilitates patient care throughout hospitalization. This role balances resource utilization with patient self‑determination, ensuring appropriate levels of care and clear documentation.

Key responsibilities

• Perform pre‑admission, admission, and ongoing stay reviews according to CMS, payer, and hospital guidelines.
• Serve as liaison among ED physicians, hospitalists, attending physicians, and physician advisors.
• Develop and implement individualized discharge plans in collaboration with the care team.
• Maintain compliance with utilization review standards and discharge planning regulations.

Required profile

• Graduate of an accredited nursing program; Bachelor’s degree preferred.
• Current state RN license and Basic Life Support (BLS) certification.
• Preferred 3 years of hospital‑based discharge planning, case management, or utilization review experience.
• Preferred certification in Case Management (CCM) or Accredited Case Manager (ACM).

Required skills

• Proficiency with computers, software, and web‑based applications.
• Basic Life Support (BLS) certification.
• Current registered nurse licensure.
• Case Management certification (CCM or ACM) preferred.
