MDS Coordinator – Licensed Nurse (Texas)

About the role

AVIR Health Group is looking for a licensed MDS Coordinator who is resident‑ and family‑focused, able to build and lead a high‑performing team, and committed to delivering accurate RAI assessments for each resident.

Key responsibilities

• Complete the Resident Assessment Instrument (RAI) process for every resident in compliance with state and federal regulations.
• Transmit and verify receipt of assessments with the state authorities.
• Develop and maintain a culture of quality, safety, and teamwork among staff.
• Supervise and mentor nursing staff involved in the assessment process.

Required profile

• Valid Texas Nurse License.
• Current CPR certification.
• At least one year of supervisory experience in a hospital, skilled nursing facility, or related healthcare setting.
• Knowledge of long‑term care operational standards as defined in the Federal Register, Conditions of Participation, and Texas state regulations.
