Hospice RN - Clinical Team Lead

About the role

Enhabit Home Health & Hospice is seeking a compassionate Registered Nurse to lead its hospice clinical team. This position coordinates patient care, ensures compliance with standards, and serves as a key liaison among physicians, referral sources, patients, caregivers, and staff.

Key responsibilities

• Coordinate hospice services to meet accepted standards of practice and company procedures.
• Review and approve patient information for start of care, recertification, resumption of care, and evaluation visits.
• Facilitate communication and relationships between physicians, referral sources, patients, caregivers, and internal team members.

Required profile

• Graduate of an approved nursing program and current state licensure (or compact licensure).
• Minimum two years of nursing experience, including at least one year in home health or hospice.
• Knowledge of federal, state, and local regulations governing home care.
• Valid driver’s license, automobile liability insurance, and reliable transportation.
• Preferred: prior management or supervisory experience.

Required skills

• Basic technology proficiency, including operation of a mobile device.
• Familiarity with electronic medical records (EMR) systems.
