Physical Therapist (PT) – Home Health

About the role

Enhabit Home Health & Hospice is seeking a licensed Physical Therapist to provide skilled, in‑home care to patients with physical impairments. You will work directly with patients and families to improve mobility, function, and independence across a range of specialty programs.

Key responsibilities

• Assess and treat patients using physical therapy modalities to enhance mobility and daily living skills.
• Deliver specialized programs such as Balance and Fall Prevention, Spine Safety, Total Hip/Knee Replacement, and Total Shoulder Replacement.
• Provide intermittent skilled care in accordance with physician orders and the established plan of care.
• Educate patients, families, and the broader health‑care team on therapy goals and techniques.
• Operate under the supervision of the Branch Director while maintaining documentation and compliance.

Required profile

• Valid state driver’s license and automobile liability insurance.
• Reliable transportation in good working condition, capable of driving in all weather conditions.
• Current licensure as a Physical Therapist in the state of employment.
• Commitment to wearing seatbelts at all times while driving.
