Weekend On-Call Registered Nurse - Hospice

About the role

The Care Team, in partnership with Traditions Health, is seeking a weekend on-call registered nurse to join its growing hospice team in Cleveland. The on-call nurse answers calls from patients, families, caregivers and referral sources and provides assessment, advice and care after business hours, working in patients' homes and other care settings.

Key responsibilities

• Respond to calls from the answering service and voicemail in a timely way, triage them and respond by phone or with a visit
• Assess patients, plan interventions and implement changes to the plan of care with the support of the medical director or attending physician
• Document assessments, interventions, care plan changes and communications with the Case Manager
• Perform patient admission visits, patient intakes and information visits, and coordinate services with vendors and the interdisciplinary team
• Attend patient deaths, support families, contact the mortuary, advise on narcotic disposal and provide bereavement resources
• Screen referral calls, explain hospice care and schedule visits with the admin on-call
• Provide point-of-service electronic charting during visits and cover continuous care hours when assigned

Required profile

• Graduate of an accredited nursing school with a valid RN license and current BLS certification
• Minimum one year of recent professional nursing experience; hospice or home health experience preferred
• Reliable transportation, valid driver's license and auto insurance, as staff work in patients' homes
• Able to cope with emotional stress, work independently and handle occasional overnight travel and exposure to adverse weather
