On-Call Registered Nurse (Hospice)

About the role

Traditions Health is hiring an on-call registered nurse in Cleveland who responds to calls from patients, families, caregivers and referral sources between 5 PM and 9 AM Monday to Friday and 24 hours on Saturday and Sunday. The nurse provides assessment, advice and care to patients with advanced illness, works in patients' homes and other settings, and coordinates care with the case manager and interdisciplinary team.

Key responsibilities

• Answer calls promptly via answering service and voicemail, triage them and respond by phone or visit
• Assess patients, plan and implement care, and document assessments, interventions, plan-of-care changes and patient responses
• Provide comfort care and coordinate services with vendors, agencies and the interdisciplinary team
• Perform patient intake visits, admission visits and other duties within scope of practice
• Attend patient deaths, support families, contact the mortuary, advise on narcotic disposal and notify the IDT and vendors
• Provide bereavement resources and refer callers to hospice care
• Work the on-call rotation, including covering continuous care hours when assigned

Required profile

• Nursing graduate of an accredited school and a valid Registered Nurse license
• Current Basic Life Support certification and a valid state driver's license
• Minimum one year of recent professional nursing experience; hospice or home health experience preferred
• Reliable transportation, as staff work in patients' homes and may travel overnight or in bad weather
• Able to work independently, cope with emotional stress and work under stressful conditions
