Triage Nurse

About the role

Tellihealth is seeking a compassionate Triage Nurse to provide telephonic triage, assess patient needs, and guide patients through remote care pathways. You will be a key member of the patient care team, working closely with providers and in‑office staff to ensure timely, high‑quality support.

Key responsibilities

• Utilize technology to manage patient conditions, gather educational resources, and coordinate care.
• Recognize when additional communication or intervention is required and act promptly.
• Follow provider care plans, helping patients prioritize health and communicate symptoms.
• Advocate for patients by answering questions and resolving medical challenges efficiently.
• Build trust with patients and families through meaningful conversations.
• Serve as a vital link between patients, providers, and office staff.
• Use EMR systems to review visits, document actions, and liaise with the care team.
• Perform other duties as assigned.

Required profile

• Active, unrestricted RN license.
• Graduate of an accredited RN program.
• Minimum 3 years of clinical experience.
• At least 2 years of experience in a related role.

Required skills

• Proficiency with Electronic Medical Record (EMR) systems.
• Telephonic triage and patient communication.
