Bilingual Clinical Pharmacist

About the role

Scene Health is seeking a bilingual Clinical Pharmacist (PharmD) to join its Patient Solutions team. The pharmacist will work with nurses, health coaches and other clinicians to improve medication adherence through the company’s digital platform, providing medication therapy management and patient education.

Key responsibilities

• Conduct patient outreach, answer inbound calls and deliver pharmacist services as part of the Scene program.
• Perform Medication Therapy Management (MTM) services, including Comprehensive Medication Reviews (CMRs), targeted reviews, medication reconciliation and prior authorization support.
• Provide asynchronous video consultations and daily check‑ins to reinforce clinical information and guide patients into appropriate care programs.
• Identify and document issues such as unnecessary medications, dosing errors, adverse drug reactions, ineffective therapy and adherence barriers.
• Communicate clinical recommendations to prescribers, follow up on responses and ensure implementation.
• Collaborate with the broader Scene team—including nurses, health coaches, product managers and client‑facing staff—to maintain continuity of care and meet productivity targets.

Required profile

• PharmD degree and active pharmacist licensure.
• Bilingual proficiency (English and French) with strong verbal and written communication skills.
• Experience conducting MTM services and comprehensive medication reviews.
• Ability to work independently, prioritize tasks and meet performance metrics.
